iTAC Equipment Reservation Form

By filling out this form the borrower is agreeing to the timely and safe return of Equipment. Due to high usage it is imperative that all borrowed equipment is returned on the same day it is signed out.

All equipment may be picked up 15 minutes before the hour or after the hour of the scheduled reservation, and must be returned 15 minutes prior to scheduled equipment return.

All equipment must be returned by 9:30pm, each evening from Monday to Friday.

If the iTAC office is unattended please call extension 4822.

Any borrower that leaves equipment unattended may forfeit their rights to loan equipment from iTAC.
