MASK FIT TEST CERTIFICATE (renew every two years)
- All Nursing, Dental and Food Nutrition students must be tested for an appropriate qualitative mask fit test (respirator). All costs and service fee are responsibility of the student.
- You can book your mask fit test either at Peak Excellence Shop or SOS First Aid company or you can get it from your workplace or at any third party company available in your area.
- Please make sure that you get fitted for 3M N95 model as it is widely use and supplied by most of our placement agency partners such as hospitals, Long-Term Care, and other health care setting.
- Your mask fit test certificate card must clearly indicate the issue and expiry date, mask model, type and size and valid every two years. Should you have any questions, please contact us directly.
New: ParaMed Mask Fit Test and Two Step TB Skin Test Service
- ParaMed is now offering a Mask Fit Test and Two Step TB Skin Test service at the following locations below effective on March 18, 2024.
- Mask Fit Test fee rate is $45.00 dollars plus tax
- Two Step TB Skin Test fee rate is $75.00 dollars plus tax
- Please contact and book an appointment at the location closer to your area below and make sure you bring your Health Form documents to complete and sign.
